Your First Visit - What to Expect
Your fist visit is an opportunity to meet Dr. Bowen and her staff to have your area of concern evaluated. If you believe that your concern will be covered by insurance, then please bring your insurance card and any pertinent medical records. If you are unsure then contact your insurance company and determine if you have coverage for that issue.
Whether your concern is an insurance or cosmetic issue, you will need to complete your health history questionnaires on your first visit. You may need to update this information if it has been longer than a year since your last visit. New patient forms can be printed from this page and filled out and faxed or emailed in ahead of time if desired.Insurance

Insurance
We accept and file most insurance policies. However, it is the responsibility of the patient to understand the individual benefit plan they participate in. Most insurance plans have a yearly deductible to meet plus an out-of-pocket co-insurance that the patient is responsible for and is due prior to surgery.. Please review your coverage with an insurance representative.
Please be sure have available your insurance information so we can access your deductible and co-pay.
Questions about our plastic surgery fees and payment options are always welcome. We want you to be comfortable with all aspects of our plastic surgery and cosmetic surgery services we will be providing.
While our plastic surgery and cosmetic surgery services and rates will of course vary from patient to patient, a deposit is required in ordered to schedule your surgery. Payment for all cosmetic and plastic surgery procedures are due in full twoweeks prior to your scheduled surgery.
Returned Checks: A $20 fee will be charged to your account for each check returned by the bank.
Collections Policy: Our policy is to be fiscally responsible and expect you to be the same. We will be happy to work with you to respond to your financial needs. If, however, you are not cooperative with these efforts, as a last resort we will refer your account to a collection agency.
Telephone Calls: We encourage you to call us if you have any questions about your care or about services we provide. If you need to speak with the doctor and she is busy caring for a patient, she will call you back during her call-back period, so that she can give your call the attention it deserves without infringing on the other patient's time.
In Summary: We can give you the best care if you provide us with feedback on your experience with us and our office. This information helps us make changes and improve the quality of your service. Please ask us any questions that you may have, whether it concerns a procedure, policy or payment issue. We will do our best to assist you.
