Your First Visit – What to Expect
Your fist visit is an opportunity to meet Dr. Bowen and her staff to have your area of concern evaluated. If you believe that your concern will be covered by insurance, then please bring your insurance card and any pertinent medical records. If you are unsure then contact your insurance company and determine if you have coverage for that issue.
Whether your concern is an insurance or cosmetic issue, you will need to complete your health history questionnaires on your first visit. You may need to update this information if it has been longer than a year since your last visit. New patient forms can be printed from this page and filled out and faxed or emailed in ahead of time if desired.
